Application forms are available here for births, deaths and marriages.  Download the relevant form and lodge it with us in person, by mail, fax or as a scan attached to an email sent to This email address is being protected from spambots. You need JavaScript enabled to view it.  All details must be completed, using the indexes to births, deaths and marriages which are available online at FreeBMD and also through several commercial websites.
